Position Summary

Registered Nurse, Reactivation Care Centre Unit 4 & 5

Your goal in this position is to provide excellent patient and family driven care, in a supportive and culturally sensitive manner. You will do this by collaborating with an interdisciplinary team of care providers, with an emphasis on the assessment, education, prevention, maintenance, and restoration of patient health. At North York General Hospital, we continue to advance the important role we play as we advocate for healthcare initiatives in the diverse community we serve. North York General Hospital’s (NYGH) Reactivation Care Centre (RCC) is an interprofessional, collaborative care model that is designed for patients who no longer require hospital care but need restorative care to support their transition home or to another setting, like long-term care. North York General’s RCC is located at Finch Site (formerly known as Branson) at 555 Finch Ave West, newly revitalized space for patients, caregivers, staff & physicians.

At North York General Hospital we continue to advance the important role we play as we advocate for healthcare initiatives in the diverse community we serve. In December 2017, the Ministry of Health and Long Term Care (MOHLTC), in partnership with multiple hospitals in the Greater Toronto Area, opened the Reactivation Care Centre – A Central LHIN Hospital Collaborative. North York General is one of the occupying hospitals at this site and as such, we are hiring Registered Nurses to work in our Alternative Level of Care (ALC) unit.

On a practical level, you will

  • Demonstrate the ability to function to the full scope of nursing practice per the College of Nurse of Ontario
  • Communicate effectively (written and verbal) with patients / clients, families and other members of the health care team
  • Think critically, set priorities and organize work in response to client / patient needs.
  • Demonstrate knowledge of transfers and lifts
  • Be committed to ongoing learning, professional development, and continuous quality improvement
  • Assume a leadership / charge role with demonstrated evidence of self- motivation regarding continuing education in areas of patient assessment, patient education and related Clinical Specialties
  • Foster good public relations and working relationships in a family centered care environment
  • Demonstrate your ability to perform appropriately in emergency situations
  • Qualifications

  • Must obtain and provide to the Hospital a current Police Vulnerable Sector Check
  • Diploma in Nursing, Baccalaureate Degree in Nursing
  • Registered and in good standing with the College of Nurses of Ontario
  • Current Basic Cardiac Life Support (BCLS) certification
  • Minimum 2 years of recent Medical / Surgical and / or clinical nursing experience working with older adult, dementia and cognitively impaired population preferred.
  • Recent health assessment course
  • CNA certification in Geriatrics, an asset
  • Knowledge of Orthopedics or Rehabilitation nursing and / or Rehabilitation Nursing CNA certification preferred
  • Bilingualism in French / English an asset
