The Opportunity

The Nurse Practitioner will work collaboratively on the delivery of resident care to meet individual needs within his / her level of competency. This role provides quality and safe resident care which helps to support and align resident needs.

What Is In It For You

• Collaborate and liaise with the MD, DOC and interdisciplinary team in care delivery.
• Assess, diagnose, and evaluate acute and chronic illness and other changes in resident’s conditions, prescribe treatments, diagnostics and follow up care.
• Support and advocate for residents’ best interests while engaging with the resident / family / substitute decision maker (SDM) regarding Care.
• Participate in resident care conferences.
• Provide clinical leadership to the LTC team members centered around the resident care needs which can include resident move in / out, hospital discharge planning, palliative and end‑of‑life care, pain management, dementia care and responsive behavior management.
• Participate in identifying, analyzing, and interpreting trends in resident care and professional nursing practice to determine priorities for educational programming for team members, residents, and families.
