If you are interested in working within an inter-professional team playing a key role in linking the needs of Rural Home Care clients with appropriate health and community services, then this Registered Nurse position will be of interest to you. Based on the Continuing Care Case Management Competency Framework, you demonstrate ability in the areas of personal and professional accountability, systems thinking, facilitating creative thinking, communicating and building relationships. Using a person-centered approach, you will develop a care plan supporting the client/family to identify needs, focus on strengths, set achievable personal health and self-care goals and provide direct nursing care to individuals across the life span. In collaboration with the client/family, you will ensure services are provided and modified on an ongoing basis, based on client’s response to interventions, incorporating stewardship into service authorization. Please note that this position supports the communities 40km SE of the Calgary City Limits.

Description:

As a Registered Nurse (RN), you will provide a wide variety of nursing services to patients, families, communities and populations, while taking necessary steps to ensure their safety and well-being. In your role, you will utilize nursing processes, through critical thinking, problem solving and decision making, as well as teach, counsel and advocate on behalf of patients and their families. You will play a leadership role in supporting an integrated and holistic approach to patient care, health promotion and maintenance. You will provide safe, quality patient and family centered care while reflecting the shared vision and values of AHS. PLEASE NOTE: Provisional Permit holders will be paid $ - $ per hour. Upon obtaining a full licence, the rate of pay will be adjusted to the applicable Registered Nurse rate in accordance with UNA Article or (ii) as applicable.

Completion of an accredited nursing education program. Active or eligible for registration and practice permit with the College of Registered Nurses of Alberta (CRNA). Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP) certification. If you have not practiced as a Registered Nurse within the past five (5) years, completion of the following free courses is required prior to employment: CRNA Medication Management (Chapters 1-4): NextGenU Health Assessment Resources Certification:

Handling, moving and lifting heavy or awkward equipment and/or individuals are required. Ability to perform all duties in accordance with related legislation. Intermediate level of computer skills. Excellent organizational and time management skills with strong interpersonal skills.

Baccalaureate degree in nursing preferred; however, skills and experience relevant to nursing practice in community sector will be considered. Minimum of two years current nursing experience with one year in a community-based setting preferred. Recent experience working with acute clients and/or long term clients with a focus on chronic disease management. Home Care experience preferred.
