About the Opportunity
As part of Primary Health Care & Chronic Disease Management Clinical Services (PHC-CDM), Primary Care Clinics (PCCs) provide in-person care for Nova Scotians who do not have a regular primary care provider, along the continuum of services that include VirtualCareNS, and other enhanced primary care access options for Nova Scotians.
Reporting to the Health Services Manager or Health Services Lead and working under the supervision or direction of regulated health professionals, the Care Team Assistant (CTA) is an active member of the collaborative health team in the primary health care setting.
The CTA provides direct care and support based on the needs of the patients seen in the clinic, served within a community health network.
As a member of the collaborative care team, the CTA is responsible to support communication with patients and families, infection prevention and control, mental health and wellness, safe handling and mobility, personal care to support procedural interventions with patients, and safety protocols.
The CTA also reinforces patient and family teaching according to established guidelines and the plan of care.
About You
We would love to hear from you if you have the following :
- Completion of the Continuing Care Assistant Program*
- Provincial Certification with Department of Health and Wellness
- Candidates are required to submit their certification and be registered and active with the CCA Registry. How-to-Sign-Up-for-the-CCA-Registry.

- Current Basic Life Support Certification
- Demonstrates effective listening and communication skills
- Competencies outlined in the CCA Competency Framework , including :
- Accountability and integrity
- Care setting management
- Infection prevention and control
- Mental health and wellness
- Nutrition, and
- Personal care, safe handling & mobility, and safety protocols.
- Demonstrates a patient-centered approach and be an advocate for improving and optimizing patient experiences and outcomes.
- Competencies in other languages considered an asset
About You Continued
Where there are no applicants with provincial certification, the employer may consider candidates who are eligible to write the CCA knowledge exam and obtain certification within 1 year from their date of hire.

Hours
- Permanent full-time position / 75 Hours Bi-weekly
- Shifts may include evenings, weekends and holidays
Compensation and Benefits
$23.70 - $25.71 Hourly
Successful candidates may be eligible for our benefits package which includes health, dental, travel, long-term disability, and life insurance coverage as well as a defined benefit pension plan.
