Lifemark’s Home and Community Care division, Community Advantage Rehabilitation (CAR) is looking for Registered Occupational Therapists to provide services to our Adult clients within the community. The position can be a flexible one, with working hours ranging from part-time to full time depending on your desired client caseload size.

OT Responsibilities :
Provide ministry funded, community based Occupational Therapy services within the client’s place of residence and / or through virtual servicesProvide occupational therapy assessment (through interview, standardized and informal assessment techniques) in the areas of home safety, home modifications, equipment prescription, fall prevention, mobility, cognition, stroke, etc.Develop and implement best practice and evidence-based occupational therapy intervention in collaboration with clients / caregiversAssign patient care plan, and provide supervision and monitoring of occupational therapy assistants when appropriateEffectively communicate information by providing education to client / caregivers and linking to community agencies / resources when appropriateComplete funding applications as applicableComplete and maintain current and accurate documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and COTO standardsOpportunity to service private clientsRequirements :
Registered Occupational Therapist with the College of Occupational Therapists of Ontario (required)Experience working within a community setting is an asset but new graduates are welcomeAbility to work independently and as part of an inter-professional teamAbility to prioritize, manage self-scheduling and meet reporting deadlinesExcellent oral, written and report writing skillsProficiency with computersStrong interpersonal and organizational skillsReliable transportation is required for this positionVisit us on -
