Job description

Position Title : Licensed Clinical Social Worker - Case Manager

Location : 100% Remote (AK, AR, AZ, CO, IA, ID, MN, MO, MT, NV, NM, OR, SD, TX, UT, WA, WI & WY only in these states)

Duration : 6 months with possible extension

Schedule : 8 am to 5 pm Arizona time (may require adjustment to accommodate different time zones)

Pay range : $86K - 92K, Annually (Based on experience, and geographic location)

Job Summary

The LCSW - Case Manager provides case management services for healthcare beneficiaries, focusing on comprehensive care coordination and resource utilization to support positive patient outcomes. This role involves screening referrals, conducting thorough assessments, developing individualized care plans, and monitoring care effectiveness. The Case Manager is responsible for handling authorization and referral requests in line with policy and ensuring care plans are within the scope of practice based on state licensure.

Key Responsibilities

Screen referrals for case management services and perform comprehensive patient assessments.

Develop and implement individualized care plans tailored to meet the needs of each patient.

Monitor and evaluate the effectiveness of care plans, adjusting as necessary to achieve desired outcomes.

Conduct regular reassessments to ensure patient needs are continually addressed.

Review and process authorization and referral requests according to established policies and guidelines.

Stay informed on the scope of practice regulations relevant to the Case Manager’s state of licensure.

Education & Experience Requirements

Active, unrestricted LCSW license in the state of practice.

Prior experience in case management, care coordination, or a related field within healthcare settings.

Two years full-time equivalent of direct clinical care to the consumer

Knowledge of state and federal regulations related to healthcare case management.

Ability to obtain a URAC-recognized certification in case management within four (4) years of hire

Preferred Qualifications

Experience working with military or government health programs is a plus.

2 years case management experience

URAC-recognized case manager certification

Proficiency in using electronic health record (EHR) systems and case management software.

Skills & Competencies

Strong communication skills, with the ability to collaborate effectively with patients, families, and interdisciplinary teams.

Excellent organizational skills and attention to detail.

Ability to work independently in a remote environment while maintaining productivity and accountability.
