As a Care Manager, you’ll deliver exceptional in-home care, build strong client relationships, and collaborate with a multidisciplinary team to ensure positive outcomes. This role combines in-office, hybrid, and community fieldwork. Key Responsibilities : Create and update personalized Care Assessments. Conduct supervisory visits and ensure quality care. Educate clients and adapt care based on risk issues. Lead and support caregiving teams. Collaborate with Scheduling to manage client coverage. Conduct Caring Consults and aim for an 80% conversion goal. Maintain accurate documentation and ensure compliance. Requirements : Bachelor’s Degree or Nursing Diploma. RPN certificate and CNO license. 1-2 years of nursing experience preferred. Valid driver’s license and car. Proficient in G-Suite and Microsoft Office. Work Schedule :  Full-time, Monday to Friday, with on-call as needed.
