Yarmouth Medical Home is attached to the Yarmouth Regional Hospital (YRH), they have a team-based, patient-centered and accessible vision for primary care which is aligned with the Patient Medical Home model of primary care. Physicians in this practice will offer urgent care access and support the Dalhousie Family Medicine Residency program that is based out of the clinic. Successful candidates may participate in teaching with Family Medicine residents and medical students through Dalhousie University.
YRH has developed into a vast health care complex serving the 58, people of Shelburne, Yarmouth, and Digby Counties. It has inpatient beds and provides a range of primary and secondary services through in-patient, outpatient and satellite programs :

Responsibilities
ensure the delivery of comprehensive primary health care services to patients of all ages and stages of health or illnessbe accountable for appropriate workload and practicesupport the Dalhousie Family Medicine Residency programprovide improved access of care by participating in urgent care rotationwork with other providers to provide after-hours access and emergency coverage when requiredcontinuum of patient care with health care providers within your practice and / or communityprovide care and coordination of community-based care, secondary and tertiary level heath carebuild to capacity within the first two yearsoperate within the Bylaws and Rules and Regulations of Nova Scotia Health Authoritywork in collaboration with other team members in the practice to best suit the community’s health needsparticipate in quality improvement and evaluation activitiesutilize an electronic medical record (EMR) and record all relevant patient details in a provincially approved EMR which is accessible to all collaborating team membersExpected Hours
