Job description

A community health organization in Central Ontario is seeking a Community Independence Care Coordinator. This full-time temporary role requires an experienced Registered Practical Nurse responsible for assessing patient needs and coordinating care plans. The ideal candidate will have strong communication and collaborative skills, as well as experience in the health care sector.
