JOB OPPORTUNITY - Manager Intake Crisis & Outpatient Services - Full-Time Permanent Position

Position Summary

The Manager Intake Crisis & Outpatient services (including the Aftercare program) will ensure that Youthdale values are reflected in the ongoing triage of services to identified need transition and continued recovery of the children youth and young adults to less intensive child or adult mental health services and supports resources and family and community reintegration. The Manager will ensure the operational functioning of these programs and will provide leadership and supervision to their staff. The Manager will also be responsible for identifying service gaps and implementing quality improvement initiatives aimed at clinical improvement.

The Aftercare team is responsible for supporting safe transitions from Youthdale to less intensive community services or back to their home environment. Aftercare will support services of brief therapies provided to client and caregivers focusing on reinforcing the skills developed during their treatment within the intensive services at Youthdale.

The Crisis team is responsible for providing 24/7 crisis responses to Children and Youth. Responsibilities include monitoring the crisis line using de-escalation techniques over the phone completing triaging assessments and navigating access to appropriate services. The Crisis Team is also responsible for guiding access to Youthdales Secure Treatment Unit and Transitional Age Unit.

The Intake team is responsible for transitions into Youthdale services (ASU TAU SU/SD LIT & DT). The team is responsible for ensuring all Licensing documents are completed and filed accordingly as well as would be liaising with legal departments and different court systems to support transfer into our secure treatment program.

The Outpatient program would support access to psychiatry consults and needed follow-up. The Manager would be responsible for the design and implementation of this new operation and would work to build partnerships with family health teams within the GTA.

As the leader of Intake Crisis and Outpatient services the Manager will be committed to implementing innovative team-based models that support evidence-based care practices. The Manager will ensure that services are reflected based on identified need utilizing standardized tools and processes are time-limited and evidenced based and that appropriate transitions occur. Part of the role will also be to strengthen community partnerships and partake in outreach and advocacy.

Location: 229 Yonge Street
Hours: 37.5 hours per week
Status: Full-Time Permanent
Union/Non: This is a non-unionized position
Salary Range: $90000 - $105000 per year

Accountability

This position reports directly to: Director Clinical Programs

Supervises: Intake Admission & Case Coordinators Crisis Support Workers Outpatient & Aftercare staff

Corporate Responsibilities

  • Works flexible hours including evenings to facilitate client meetings and debriefs where required.

Major Responsibilities

  • Identify and address any challenges or opportunities connected to the delivery of aftercare and crisis services.

  • Identify and address any challenges or opportunities connected to the delivery of aftercare and crisis services.
  • Support the team to recognize clients complex needs and assist the staff in developing individual care plans with customized components and novel addition assist the team with identifying challenges and working with them.
  • Support intake and triaging streams and arrange clinical conferences when challenging clinical concerns arise
  • Ensure appropriate training is developed for frontline staff and that staff are delivering on responsibilities
  • Ensuring supervision is provided to frontline staff and concerns are addressed in a timely manner including performance related concerns
  • Liaise with child and adult community service providers to build networks of care.
  • Ensure that members of the multi-disciplinary team understand each others roles and expertise.
  • Attend and provide leadership in various forums such as leadership meetings team meetings case conferences plan of care client progress meetings and quality improvement meetings.
  • Ensure that support supervision evaluation and training are provided to staff.
    Foster positive interpersonal relationships with staff residents visitors outside agencies government officials and related professional associations.
  • Ensure compliance to policies and standards of professional practice with respective regulatory bodies (e.g. Ontario College of Social Workers and Social Service Workers College of Registered Psychotherapists Auditor Accreditation etc.).
  • Maintain and adhere to all relevant legislation organizational and occupational health and safety policies and procedures and the mission vision and philosophy of the agency.
  • Understanding the CYFSA and ministry licensing requirements as it relates to Child and Youth Service

Qualifications

  • Masters degree in nursing psychology social work psychotherapy or relevant field.
  • Regulated Health Care Professional and registration with the Ontario Nurses Association College of Registered Psychotherapists of Ontario OCSWSSW or related professional association
  • At least three years of supervisory experience in a related field preferably managing and supervising interdisciplinary staff. And 5 years of practice experience
  • Knowledge of community resources within the GTA and step down interventions
  • Knowledge of children and youth mental health and the social determinants of health.
  • Expert knowledge of system navigation and referral pathways to community mental health addiction support health social agency resources etc.
  • Strong interpersonal and communication skills with a proven ability to establish positive partnerships and work as a lead member of a multi-disciplinary team.
  • Ability to communicate verbally and write clear concise reports and documentation.
  • Clinical experience is required to support decision-making i.e. escalation or changes in service levels.
  • Experience with multi-disciplinary collaboration/communication.
  • Experience facilitating transitions from treatment services and/or between hospital/LIT and the community.
  • Experience with intensive case management teams and crisis teams are considered assets.
  • Experienced with standardized triage tools and assessment tools
  • Capacity to integrate research into practice and create utilize and/or disseminate new knowledge.
  • Knowledge of Quality Improvement Initiatives
  • LEAN training would be an asset
