About the Role
The Licensed Practical Nurse (LPN) reports to the Director of Care and / or designate. This position is the role model and team leader to an assigned unit, and responsible for coordinating care delivery of the persons served.
The LPN performs duties and monitors departmental activities in accordance with their scope of practice, established standards, regulations, policies and procedures to maximize individualized and resident-centered care.
Responsibilities :
- Include but not limited to;
- Assesses residents’ needs; implements and evaluates individualized resident-centred care plans.
- Ensures residents rights are observed and adheres to a holistic philosophy of wellness.
- Liaises with physicians, community agency representatives, families, visitors, volunteers, other disciplines and departments to ensure quality resident-centered care is delivered.
- Assesses and treats all wounds on the unit and acts as a resource to care staff for strategies to prevent skin breakdown.
Consult with RN or designate for changes in treatment strategies as needed.
- Administers prescribed medications and treatments within the limitations of the BC College of Nurses and Midwives (BCCNM).
- Monitors and reports changes in physical and psychological conditions of residents. Document changes on the resident health record, notifies family and physician of changes in condition.
- Active participant in Interdisciplinary Care Conferences.
- Participates in care plan reviews to ensure current and individualized care plans.
- Supports the administrative requirements at the community in accordance with established standards, including but not limited to;
admissions, discharge and ensuring confidentiality is maintained at all times.
- Actively participates in orientation of new staff, community programs, meetings, committees and educational in-services.
- Provides coaching and direction to care staff as needed and uses in the moment education to improve resident care which includes completing all Point of Care (POC) documentation.
Seeks assistance / support from RN or designate as needed.
- Maintains an up-to-date knowledge of community policies and procedures, particularly those concerning emergency procedures including equipment management, guidance and assistance on safe work practices.
- Performs other related duties as required.
Qualifications and Skills :
- Graduate of an approved program for Practical Nursing and holds a current license with the British Columbia College of Nurses and Midwives (BCCNM) as an LPN.
- Valid Occupational First Aid Level 1 certification.
- Successful annual Resident Assessment Instrument (RAI) competency testing.
- Previous experience in a long term care facility an asset.
- Demonstrates collaborative and team working skills.
- All successful applicants must pass the vulnerable sector Criminal Record Check applicable to Provincial guidelines (see BCCNM license).
- Ability to read, write, and speak English proficiently and understand verbal and written instructions.
- Computer skills including the ability to navigate basic computer programs such as Word, Excel, Outlook.
- Must be in good health, physically able to perform repetitive tasks, lift, squat, bend, stand and walk as required to perform the duties of the position.
