Through a new, innovative partnership with Nova Scotia Health (NSH), we are opening the first transitional health center in our province, in West Bedford. Shannex is the operating partner for the transitional health centre on behalf of Nova Scotia Health. you will have an opportunity to work in a facility with a purpose and design dedicated to providing care to NSH patients in central zone, who no longer require hospital care, but need services before they can safely return home / community.

We are searching for a Full Time Transitional Care Assistant (CCA) to join our Transitional Health Facility located in West Bedford, Nova Scotia.

The transitional health model supports NSH patients / families who require additional time, services, and support to transition back to home / community, by providing an alternate space with intentional programming, building design, and resource allocation to increase the probability of returning to their previous living situation.

A Continuing Care Assistant (CCA) at West Bedford Transitional Health provides personal care and assistance with activities of daily living through the established philosophy of service, supporting the maintenance of the patient’s functional abilities with the goal of returning home.

About the Opportunity

  • Encouraging, motivating, and assisting patients with hygiene and daily living activities, such as : bathing, dressing, oral care, skin care, meals, and mobilizing, according to the patient’s care plan
  • Collaborates with the patient and other members of the interdisciplinary team in the development of individualized patient Care Plan
  • Assists in the planning and implementation of patient leisure activities on a daily basis
  • Assists with light housekeeping duties
  • Performs delegated nursing acts according to company policy and scope of practice
  • Communicates and reports relevant information regarding patient care or safety to Supervisor.
  • About You

  • You have a valid Continuing Care Certification or Provincial Personal Care Worker course or equivalent;
  • You are registered in Nova Scotia’s CCA registry as per the Continuing Care Assistants Registry Act
  • You can work a schedule of flexible hours and shifts inclusive of days, nights, weekends, and holidays,
  • You can provide a clear criminal record with vulnerable sector screening;
  • Alzheimer’s and Dementia Care course considered an asset;
  • Previous working experience with elderly in long-term care or a senior living environment an asset
  • Valid CPR & First Aid Certification preferred.
  • Asset : Previous experience supporting the care of vulnerable populations, with complex social situations, who are transitioning across health care settings, with a goal of home first.
