About Us
The Hamilton Family Health Team (HFHT) is seeking a dynamic and experienced Manager , Clinical to lead and support high-quality, patient-centered care across our primary care teams. This leadership role is integral to advancing HFHT’s mission, vision, and strategic priorities through effective program coordination, professional practice leadership, and system-level collaboration.
The Manager, Clinical will support practice-related support for nursing, physician assistants, nurse practitioners, midwives and some HFHT interprofessional staff working across the practice sites, ensuring clinical quality and performance improvement while fostering a supportive, inclusive, and high-performing work environment.
