Position Summary
The Primary Health Nurse II provides health education and care within the scope of nursing practice. As a member of a client-centered interdisciplinary team the Primary Health Nurse II provides care to individuals promoting wellness prevention of disease and/or disability and the management of chronic diseases/conditions. The incumbent works as an advocate on behalf of the client to ensure the appropriate links/referrals to other human services that would enable the client to achieve their health goals.
Qualifications
Education
Graduate of approved Registered Nursing education program BN preferred.
Must be a member in good standing with CRNM
Skills Expertise and Experience
A minimum of five (5) years experience working as a Registered Nurse
A minimum of three (3) years experience working in a community health setting
Knowledge and Experience working with Students and/or Volunteers
Current CPR Certification at Basic Rescuer Level
Critical Skills
Proven knowledge and ability in critical thinking decision-making problem-solving and crisis management

Demonstrated ability to prioritize and focus work-related activities to effectively and efficiently achieve service goals and produce expected results/outcomes

Communication
Excellent verbal and written communication skills and proficiency in the use of computer systems including Microsoft Office suite

Strong working relationships with Community Resources
Fluency in additional languages relevant to the client population
Experience working with service specific programs including EMR eChart and Accuro
