ROLE PURPOSE
This position will provide outreach support to client(s) referred to Connective by our funders. You will work to enable clients’ adjustment to and success with life in the community.
STATUS AND CLASSIFICATION
Unionized. This position is open to applicants of all genders.
Monday to Friday, 0900 - 1700

REPORTING AND WORKING RELATIONSHIPS
Reports and is accountable to the Program Manager or their designateParticipates as a member of Connective team to ensure quality and continuity of services to clientsLiaises with staff and volunteers of community and government agencies as requiredLiaises with Connective staff as necessaryAccepts clinical direction from the care teamParticipates as a member of Connective staff teamKEY ACCOUNTABILITIES
Receives referrals from fundersWorks with the client, researches background material, and seeks the input of other professionals and family members, to provide the assessment that will ascertain needs and postulate intervention possibilitiesWorks with the client to consider issues and creates a mutually agreed intervention and support plan that has measurable short and long‑term goalsEnsures issues of daily living, including but not limited to housing, nutrition, clothing, hygiene, leisure lifestyle and access, employment, and education are addressed in this planEnsures the client’s ongoing legal, mental, and physical health issues are addressed in the planWhen and as needed, provides a wide range of interventions to enable the client to address and accomplish the established goalsInteracts with other professionals, family members, and organizations to ensure services, admissions, and treatments are in the client’s interest and supportive of the long‑term goalsOn the client’s behalf, is a resource to other professionals, including but not limited to the courts, probation services, and community and institutional psychiatric servicesMaintains extensive records and case notes in accord with Community Living British Columbia policies and standardsCompletes monthly progress reportsParticipates in staff and resident / staff meetings as instructed by the Program ManagerOther duties as subsequently negotiatedWORKING CONDITIONS
Functions independently, while remaining a crucial member of the care teamAdheres to Connective policies and proceduresIs available to work flexible hours as needed, which may include some evenings / weekends, as determined by program / client needs and schedulesIs a member of CUPE Local 1936‑11Receives benefits and works within the working conditions as outlined in the collective agreement and personnel manualEXPERIENCE AND SKILLS
Graduate with a Diploma or Certificate in Social Services and one‑year of relevant experience is preferred; in‑house training can be providedCounselling experience in the appropriate field and demonstrate a clear ability to respond well in crisis situationsExperience with clients with a history of conflict with the law, developmental disabilities and / or spectrum disorders, interaction with social agencies and experiencing psychiatric / personality disordersUnderstanding of providing service to people with different learning abilitiesExtensive knowledge of community services in Lower Mainland and Fraser Valley as they relate to persons with psychiatric involvementExperience in assessment, case / care plan development and implementationDemonstrated an ability to deal with potential violenceDemonstrated an ability to develop programs tailored to the particular diverse needs of the individualExcellent written and verbal communication skills in EnglishREQUIREMENTS
Ability to successfully complete a Criminal Records CheckCurrent Emergency First Aid CertificateClass 5 driver’s licensePersonal transportation that is in good repair, insured for business up to $1,000,000 for liability, and which can be used to transport clientsCOMPENSATION
As Connective is a unionized environment, the starting wage is $27.54 per hour. The wage will increase with every 2,000 hours worked up to a maximum of $31.61 per hour.
Important : Candidates must be legally authorized to work in Canada, observe their weekly hours limit if under a temporary or study visa, and provide proof of eligibility if selected for the role.
