The Licensed Practical Nurse (LPN) in Home Care works as a member of an interprofessional health care team utilizing the nursing process to provide direct nursing care to individuals across the life span. Working as a Case Manager, the LPN is accountable for assigned caseloads with the primary responsibility to assess client needs, determine service needs, negotiate service options, care planning, and coordination of transitions across care settings. The LPN also performs interventions and restricted activities for clients as required according to the client need. There is also an expectation to be a mobile worker. This role requires travel up to 100 km / day caring for clients in their homes or in Rural community. You will be responsible to operate a laptop computer and mobile phone. Standing, walking, lifting, pushing, pulling, stooping and carrying equipment is required throughout the shift.

Required Qualifications

• Completion of an accredited practical nurse education program.
• Active or eligible for registration and practice permit with the College of Licensed Practical Nurses of Alberta (CLPNA).
• Current Basic Cardiac Life Support - Health Care Provider (BCLS-HCP).
