Job Summary
13 Week Contract
Hours : 3 : 00 PM to 11 : 30 PM
The Licensed Practical Nurse (LPN) provides hands-on patient care under the supervision of a Registered Nurse (RN). The LPN works collaboratively as part of a healthcare team, taking responsibility for patient care tasks to support positive health outcomes.
Conducts data collection on patient health, focusing on physical, emotional, and cognitive well-being, ensuring ongoing, methodical assessments.
Assists with hygiene, mobility, and meals as directed by the RN.
Provides assistance with patient positioning and supports care in pre-operative, post-operative, and end-of-life situations.
Ensures a safe and comfortable environment by responding promptly to call lights and notifying the RN of any changes in the patient’s condition.
Adjusts care delivery based on the individual needs of patients, considering factors such as age, cultural background, language barriers, and sensory impairments.
Communicates effectively with patients, their families, and caregivers in a way they can understand.

The role involves frequent interaction with hospital staff, patients, and visitors.

Required :
Longterm Care Experience (Required)
Subacute Care Expereince (Required)
Education
Required :
Completion of an accredited Licensed Practical Nursing (LPN) program.
Graduate of an accredited program for LPN
Certifications & Licensure
Required :
Active LPN license in New Jersey.
Current BLS / CPR certification from the American Heart Association (MANDATORY) .
