About the Role
The Assisted Living Clinical Manager reports to the General Managerand / or designate and is the role model and team leader who coordinates theactivities for Home Support Services.
This role will assess and liaise with residentsand ensures that the services delivered reflects the Personal Service Plan(PSP) of the resident.
This role will also ensure that all persons served inAssisted Living and Independent Living receive quality care.
Responsibilities
Included but not limited to :
- Engage and mobilize your team to achieve thepurpose, goals, objective and key results of the organization.
- Be an engaged collaborative team player,developing and maintaining professional relationships that support andstrengthen the organizational culture and purpose.
- Model the values and leadership behaviours of organization and inspire other to do the same.
- Assesses residents against the criteria forAssisted Living for Seniors and Persons with Disabilities.
- Pre-admission and Admission assessment on allresidents choosing to reside in the community. This includes Assisted Livingand Independent Living residents.
- Liaises with the applicable Health AuthorityCase Manager during the admission process of a new resident and on an ongoing basisreviews the resident’s status and meets with the Case Manager.
- Collaborates with other leaders on delegation oftasks for Private Pay Assisted Living residents.
- Reviews all incident reports and mealabsenteeism forms and follows up as appropriate.
- Develop, communicate and update PSP with HomeSupport Workers.
- Monitors the administration of prescribed medications and treatments completed by the Home Support Workers
- Ability to administer medications and treatments within the limitations of British Columbia College of Nurses and Midwives (BCCNM).
- Ensures the Home Support service provided,respects and promotes the resident’s right to independence.
- Discuss with the resident any noted change topersonal care needs and liaises with other departments to communicateamendments to the PSP if applicable.
- Actively participates and facilitates communityprograms, meetings, committees andeducational in-services.
- Maintains documents according to establishedstandards, regulations, policies and procedures, ensuring confidentiality ismaintained.
- Recruits qualified staff and arranges theirorientation and ongoing education. Provides direction and support to staff intheir daily physical, psychological and social care of residents.
- Evaluates home support staff performance andmakes recommendations for improvement where indicated.
- Encourages and adheres to a holistic philosophyof wellness, recognizing that each resident is independent and is capable ofmaking decisions about their life and these decisions will be respected.
- Will collaborate with the resident or substitutedecision maker to develop a Transition Plan, when the resident is no longerable to live independently.
- Maintains a safe environment for residents,family and staff, through : accident prevention, knowledge of emergency proceduresand regulations, the safe use of supplies andequipment and reporting when repairs are necessary, and utilizing infection controlprocedures.
- Performs all other duties as required.
Qualifications and Skills
- Graduate of an approved program for PracticalNurses and holds a current full scope License with British Columbia College ofNurses and Midwives (BCCNM).
- Knowledge and understanding of the AssistedLiving Regulations, Assisted Living Registry, and Community Care and AssistedLiving Act.
- Valid First Aid Certificate.
- Two years’ recent experience as a practicingLPN, preferably in Assisted Living or long term care setting that included thedistribution of medications.
- All successful applicants must pass thevulnerable sector Criminal Record Check applicable to Provincial guidelines.
- Ability to read, write and speak Englishproficiently and understand verbal and written instructions.
- Ability to effectively interact with the variousindividuals involved in providing holistic care in a flexible resident focusedenvironment.
- Ability to operate related equipment.
- Intermediate computer skills in MicrosoftWindows environment.
- Excellent interpersonal and coaching skills.
- Ability to motivate and lead others in theaccomplishment of a task.
- Knowledge and experience with managing others,employment practices, and health and safety practices and policies.
- Must be in good health, physically able toperform repetitive tasks, lift, squat, bend, stand and walk as required toperform the duties of the position.
Special Requirements :
- Must be willing to work the hours necessary to ensure the effective operation of the community.
- Must have the capacity to deal with physical and mental impairment, death, and dying.
- Must be prepared to respond to emergentsituations.
