Clinic Nurse Care Coordinator Supportive Housing
1 Full-Time Position
We provide primary health care through a range of options including prevention, assessment, diagnosis and treatment. Our interprofessional teams include health professionals, educators and support workers representing both traditional Anishinaabe and contemporary models of care and using wholistic health and community development concepts.
The Clinic Nurse / Care Coordinator is part of a client-centered professional team structure and reports to the Supportive Housing Program Coordinator. The clinic nurse / care coordinator works with a variety of registered and non-registered health care providers, both directly and under the facilitation of medical directives, to facilitate coordination of client care and support optimal functioning of the team to provide health care services.
This position will support our Supportive Housing Program that is in collaboration with Ontario Aboriginal Housing. This program will use a cultural approach to help people attain housing. With an Indigenous approach, cultural assessments, and medicine wheel teachings, we hope to guide and help those in our program through their healing journey by providing safe housing. The program will also provide referrals to partner organizations.
NOTE : Traditional practices involving burning sacred medicines including tobacco, sweetgrass, sage and cedar and other cultural protocols occur regularly within our work setting.
QUALIFICATIONS :
Salary : $30.85-$38.91 per hour with excellent benefits, including HOOPP pension plan. Salary is in accordance with the Ontario Nurses Association Contract (ONA). APPLICATION DEADLINE : Open Until Filled
