The Assessor / Coordinator (Client Care Coordinator) provides case management services including assessment, planning, monitoring, and evaluation. The role primarily involves managing client / patient access services, serving as a single entry point, and coordinating various community-based services within SHA (Saskatoon).

Minimum Education and Qualifications

• Bachelor of Science in Nursing (BScN) or acceptable equivalents such as Bachelor of Social Work, Bachelor of Occupational Therapy, Bachelor of Physical Therapy, Bachelor of Nutrition.
