Position overview
- Use sound judgment and apply critical thinking in providing resident care to predominantly older residents in a Long Term Care setting, independently and as part of an interdisciplinary team.
- Demonstrate expertise and leadership in nursing practice; coordinate the implementation of clinical interventions and evaluation of resident outcomes based on goals of care.
- Act as a resource for the interdisciplinary team by providing information and coordination of ongoing resident care.
- Support the work of RCAs and LPNs and promote cohesive teamwork to ensure resident-centered care.
- Enhance quality of life of residents and families.
- Consult, confer, and collaborate with other health care providers.
- Maintain and advance your own clinical competence.
- Participate on designated committees / teams and approved research projects as assigned.
Education & Experience
Current practicing registration as a Registered Nurse or Registered Psychiatric Nurse with the British Columbia College of Nurses and Midwives (BCCNM).Completion of an approved gerontology course or an equivalent combination of education, training and experience.Skills & Abilities
Demonstrated ability in nursing practice related to residents with chronic diseases.Knowledge of pathophysiology of common medical conditions, aging changes, polypharmacy, treatment protocols, and nursing interventions for common diagnoses and care situations in Long Term Care (e.g., stroke, chest pain, pneumonia, pain management, wound care, delirium, dementia, depression, other mental health conditions, frail elderly with physical / cognitive impairments, chronic illnesses such as COPD and CHF, diabetes).Ability to perform nursing assessments and interventions for common diagnoses and care situations relevant to residents (e.g., respiratory symptoms, pressure ulcers, delirium, dementia, behavioral symptoms of dementia, Falls, incontinence, etc.).Ability to recognize physical adaptations required for the frail elderly and / or physically impaired residents.Experience with End of Life care approaches and embedding palliative care in long-term care, including discussions on goals of care with residents, families, and the Most Responsible Clinician.Chronic disease management : anticipate and respond to potential complications and care issues for the designated resident group.Proficiency with equipment such as lifts, oxygen, suction machines, pumps, and related devices.Ability to develop and implement individualized care plans aligned with resident and family priorities, including counselling and teaching.Skill in managing acute / chronic pain and caring for residents with cognitive impairment.Safe care and management of residents with oxygen therapy, VAC dressings, wounds, catheters, ostomies, tracheostomies, ventilator management, and related treatments.Competence in autonomous RN scope of practice and standards of care, including IV therapy, infection control, isolation protocols, outbreak protocols, nicotine replacement therapy, and acute event orders.Please note : The hours of work, days off, and work area may be subject to change consistent with operational requirements and Applicable collective agreement.
