Do you have what we're looking for?

You are available between the hours of 8AM-6PM on Monday thru Fridays AND every other weekend

You are Dependable and can maintain a Consistent Schedule

You have a valid Drivers License and Reliable Vehicle

You really Care about people

You are local to the Port Perry & Uxbridge area OR are willing to drive to Port Perry & Uxbridge

Must be willing to travel within the Uxbridge and Port Perry area

Must be ok to see client with pets; such as cats or dogs

Job Summary

The Personal Support Worker is a member of the home care team who works under the supervision of the Care Coordinator and performs various personal care services as necessary to meet the clients needs.

Responsibilities

Assistance with personal care and hygiene

Meal Preparation

Light Housekeeping

Accompany clients on outings

Shopping and errands

Medication Assist / Reminders

Ambulation and transporting of clients

Provide companionship to clients

Ability to follow care-plan

Documentation / Charting

Qualifications

PSW / DSW / SSW / HCW (or equivalent) certification from an accredited educational institution

Valid Police Vulnerable Sectors Check

Current CPR / First Aid Certification

Negative TB skin test or chest x-ray

Up-to-date Immunization Record, including two COVID-19 vaccinations

Valid drivers license and reliable vehicle is required

Access to Internet / mobile phone

Must be able to wear Personal Protective Equipment (PPE)
