The Registered Nurse on the Transitional Care team has the clinical skill and expertise to assess, plan, develop and co-ordinate care to individuals within the North Wellington area. This RN will provide treatment, using rehabilitation strategies, and support services to clients, within an interdisciplinary team model.

What you'll be doing

The Registered Nurse possesses education and critical knowledge; demonstrates critical thinking, judgment and decision making skills; integrates research into practice; and adapts nursing care to patient needs - all framed by the ethics of rights, care, and responsibility. The Registered Nurse, as an essential interdisciplinary team member, practices from a theory base to provide the full range of nursing care services. The RN promotes a safe environment and provides care to more complex patient populations whose needs and outcomes are not always predictable. The Registered Nurse at Homewood additionally specializes in mental health nursing and in the provision of competent, holistic care addressing the bio- psycho-social and spiritual needs of patients, families, the community, and coworkers.

What we're looking for

  • Licensed as a Registered Nurse with the College of Nurses of Ontario
  • Mental Health Nursing certificate preferred
  • BScN and CPMHN(C) preferred
  • 2+ years recent experience in Emergency Mental Health / Acute Assessment preferred
  • Strong assessment skills
  • Working knowledge of Mental Health Act and community mental health resources
  • Proven excellence in communication, problem solving, conflict resolution and organizational skills
  • Proven ability and skill in working independently and as a part of an innovative interdisciplinary team
  • CPR / Basic rescuer
  • Ability to prioritize and function effectively in a demanding and challenging environment
  • Highly skilled in assessment and treatment of psychiatric disorders, including PTSD, suicide prevention, and the management of aggressive behaviour
  • Current knowledge and experience specific to emergency mental health and crisis intervention and concurrent disorders
  • Knowledge and experience using CBT, DBT, and motivational interviewing techniques
  • Proven ability to assume leadership
