We are currently looking for someone to join our Rehabilitation Team in the role of Social Worker. We are actively expanding our Virtual Care Model and are looking for professionals who want to be a part of this exciting growth. This role can be located anywhere in Ontario and provides oversight to the therapy assistants working directly with patients in the home.
This is a Casual position.
HYBRID REQUIREMENTS : Please note that this role requires you to be able and willing to travel in Ontario as needed for in-person appointments with our patients.

What The Role Involves
Professional assessment of patients in the community and in their homesDeveloping and maintaining an effective Care Plan with respect to Community Health Services, independently or in cooperation with the Home and Community Care Support Services (HCCSS)Working with the patient on appropriate time-specific, measurable goals and outcomes in collaboration with the care teamWorking independently and as an integral member of an interdisciplinary care teamProvide assessment, intervention and discharge planning, within the scope of practiceDemonstrates a clear understanding of Community Resources, including fundingProviding active guidance for workplace Health and SafetyPerforming other duties as requiredWhat You Bring
Possess Master’s Degree (or equivalent) degree in Social Work from a recognized universityCurrent, unrestricted license to practice with Ontario College of Social Workers and Social Service Workers (OCSWSSW)Proven computer skills, including email, word processing, work scheduling systemsStrong assessment skills and the ability to participate in effective treatment programsProvide one-on-one therapeutic support with clientsPromotes independence of the client in the home and in the communityA reliable vehicle for some regular travelExcellent English verbal and written communication skillsClear Background and Vulnerable Sector CheckCarePartners In Your Community
In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
