You will be responsible for providing high-quality nursing care to patients in the community. You have clinical skills and knowledge of wound management infusion therapy palliative care and chronic disease management.
- Provide patient-focused safe holistic nursing care and work collaboratively with other members of the health care team
- Ensure that nursing care provided to our patients is based on best practices to achieve successful outcomes
- Continually assess anticipate identify and respond to new and changing needs of our patients
- Travel within your region between patients homes
Qualifications :
Minimum two years nursing experience (Community Nursing experience is ideal)Registered and in good standing with the College of Nurses of Ontario (CNO)Ability to work independently in the communityProficiency in English both verbal and written required (additional languages an asset)Valid Ontario drivers license and reliable vehicleExperience with infusion therapy mental health palliative care and wound careExcellent patient assessment and teaching skills#TOnurse
