Job Description

A Social Worker for Integrated Care Solutions will be able to adapt social work principles and practices to the community setting. This includes, but is not limited to, assisting the community adult client to maximize social, emotional and cognitive functioning that promote optimum independence and improve quality of life.

Duties and Responsibilities

  • Communicate with clients, families / caregivers, service providers and other professionals to collaborate and coordinate services
  • Maintain and manage a caseload in an efficient and effective manner ensuring priority code timeframes are met
  • Assess the client’s adjustments to medical, emotional, psychological or social problems and the factors that affect them
  • Utilize a client-centered approach to identify, prioritize and monitor potential psychosocial problems and goals
  • Establish, implement, evaluate and adjust the psychosocial treatment plan in collaboration with the client and / or family, caregivers, care coordinator and other community partners
  • Enhance or teach coping skills . relaxation, stress management, anger management, healthy grieving
  • Advise clients / caregivers on the availability of community resources and contact agencies on their behalf when appropriate . Federal, Provincial, Municipal and Local agencies in relation to income security, pensions, financial assistance, etc.
  • Determine when further social work intervention in no longer necessary and assist clients in planning for discharge
  • Recommend and facilitate referrals to support services within the community
  • Comply with continuous quality improvement initiatives of Bayshore Home Health
  • Developing a flexible, responsive system to meet changing and competing demands
  • Demonstrate knowledge of Standards of Practice guidelines of Provincial regulating body.
  • Liaise with other Case Managers,funders or Government & hospital programs as required.
  • Liaise with Provincial regulating body as required.
  • Adhere to Bayshore Policies and Procedures.
  • Participate in quality activities and continuous improvement initiatives in keeping with the company's Quality Management System.
  • Participates in proactive Health& Safety activities while performing all duties. Is responsible to notify immediate Supervisor of any Health& Safety risks or concerns
  • Actively participates in and demonstrates support for continuous quality improvement initiatives
  • Positive work and attendance performance essential
  • Maintains confidentiality of client and corporate information and discusses same only with appropriate Bayshore personnel.
  • Complete other tasks as requested

Job Qualification

Key Qualifications :

  • Document and report all tasks / information according to Bayshore policy
  • Maintain treatment records in accordance with the Standards of Practice of the Ontario College of Certified Social Workers and Social Service Workers
  • Use documented reports, individual discussions or case conferences to keep appropriate health care providers informed of the client’s status
  • Consult, collaborate and communicate with members of the client’s treatment team
  • Communicate with vendors, or other relevant individuals following consent and privacy legislation
  • Communicate and collaborate effectively and respectfully with others
  • Submit all clinical client reports within the pre-determined timeframes
  • Return completed charts promptly after client is discharged from services
  • Demonstrated agility in decision making; ability to think critically in order to respond to a rapidly changing environment
  • Ability to assess risk, and effectively utilize staff resources throughout all sites to support patient acuity and skill level of staff members while considering the global needs of all patients
  • Ability to listen, engage, collaborate, negotiate, facilitate and entertain creative solutions in order to enhance patient flow organizationally and mobilize teams to achieve throughput
  • Maintains positive and collaborative working relationships within the interdisciplinary health care team
  • Demonstrated ability to be self-directed and thrive in a dynamic environment
  • Able to remain calm under pressure; role model composure
  • REPORTING RELATIONSHIPS

    Communicate with the Manager of Therapy Services or designate regarding clinical issues and / or service issues. Be accountable to the Director or designate, for Integrated Care Solutions

    OTHER SKILLS & RESPONSIBILITIES

    Exceptional interpersonal skills and ability to handle difficult situations in an objective consistent format; ability to work independently and effectively lead a team;above average report writing and communication skills with both Field and Administrative employees; valid driver’s license and evidence of current CPR certification; solid knowledge of the principles, practices, and methods of community care and service delivery to clientsas well as program development, implementation and evaluation.

    Internal candidates must have a track record of achievement in dealing with clients and employees and have consistently shown above average initiative. Successful internal candidates must complete a six- month probationary period specific to the PCC role.

    STANDARDS OF PERFORMANCE

    The Social Worker must demonstrate ongoing competency in completing all duties and

    responsibilities as detailed in this job description, in responseto changes by the provincial regulating body, as well as agreed upon specific goals and objectives.

    PERFORMANCE REVIEW

    Performance will be reviewed on an ongoing basis with a formal review at the end of the six-month probationary period and subsequently, on at least an annual basis. Input will be sought from the employee, ICS team and in addition, will be based on ongoing client feedback and the extent to which performance meets expectations.

    Education

    Possess a Master's and / or Bachelors Degree in Social Work

    Qualifications and Requirements

  • Be an active member in good standing of the Ontario College of Certified Social Workers and Social Service Workers
  • Retain professional liability insurance ($5 million minimum)
  • Provide a police clearance that meets the Bayshore Home Health policy
  • Possess general computer skills and have access to a computer with appropriate password protection and internet capabilities
  • Minimum of two years of community-based therapy experience (preferred)
  • Be able to drive, hold a current, valid driver’s license and have access to a vehicle in good operating condition (preferred)
  • Must carry two million dollars in automobile insurance coverage if utilizing personal vehicle for appointments
  • Hold a current CPR certificate (Basic Rescuer )
