POSITION SUMMARY :
St. Joseph’s Home Care (SJHC) is dedicated to providing quality, compassionate, and client centered care to individuals in their homes and / or alternative settings through our various programs. Registered Nurses enhance the life of their clients by providing a broad range of care needs within the framework of SJHC’s policies and procedures and the standards of practice prescribed by the College of Nurses of Ontario. Through their work, Registered Nurses contribute to the healing and independence of their clients and promote the continual improvement of the overall health of the community.
CORE DUTIES AND RESPONSIBILITIES
- utilize the nursing process and standards of care (College of Nurses of Ontario – RN)
- perform pronouncements in keeping with RN standards
- monitor and comply with DRN documentation
- work independently in the community in the execution of daily tasks
- complete point of care assessments and comprehensive nursing assessments (eg. Mini-metal status, depression, pain, etc)
- practice infection prevention and control measures
- engage as a support network for peers also working in the community
- provide the highest quality care and participate in quality processes as required
- complete all necessary client assessments
- implement and deliver client’s plan of care, recommend care plan adjustments to Managers
- participate in the effective and safe coordination of client care among team members and managers
- ensure appropriate caseload management, ongoing monitoring and evaluation of care.
- collaborate respectfully with the client, family members, other team members
- engage other care multi-disciplinary providers as necessary (eg. Physicians, OT, PSW)
- engage effectively with external stakeholders as necessary (eg hospital partners)
- advocate for the client / caregivers in a collaborative and effective way
- perform all necessary documentation in a real time manner
- interpret information, support decisions, facilitate communications
- utilize technology in order to effectively document and communicate with other team members and stakeholders
- apply current knowledge and research to practice, share knowledge with others
- engage as Preceptor, coach, mentor to others as requested
- maintain current competencies and participate in continuing professional development opportunities
- seek to obtain advanced or specialized nursing qualification / skills to meet client care needs
- carry out self-appraisals in accordance with the College of Nurses of Ontario Standards of Practice
- complete SJHC self-evaluation and the SJHC appraisal process in keeping with standard protocol’s
- participate fully in additional required and / or mandatory training, and ongoing professional development requirements
- maintain open and effective communications with others in the organization
- work collaboratively in the best interests of the client at all times
- report any client and / or staff safety concerns, utilizing the appropriate forms or tools
- report any work tools or device issues in a prompt and informative manner
- collaborate with others in the ordering of nursing supplies and / or necessary equipment
- support referral and intake processes as requested
- perform general administrative / non-clinical duties associated with this role

BSc in Nursing, requiredAdditional nursing speciality skills (wound, palliative, foot etc), preferredCurrent and unrestricted registration with College of Nurses of Ontario, requiredPrevious experience in Home or Community nursing environment, preferredPrevious experience with surgical discharge clients, preferredExperience working in a unionized environment, an assetPrevious experience as a Preceptor, preferredIV medication administration education and / or experience, preferredBasic cardiac life support (BCLS) education and / or experience, requiredPrevious experience using mobile tool and devices in the executing of work, preferredPrevious experience with Edmonton Symptom Assessment System (ESAS) and / or Palliative Performance Scale (PPS), requiredChronic disease management and / or peritoneal dialysis experience, an assetPrevious experience with complex clients, preferredExperience with Gentle Persuasion Approach or equivalent, an asset
