Job description

DUTIES AND RESPONSIBILITIES :

  • Acting as a resource in the area of Congestive Heart Failure (CHF) to members of the interdisciplinary team;
  • Providing consultation services to patients / families, health professionals related to Congestive Heart Failure management; Facilitating interdisciplinary care by participating in regular team meetings, providing a linkage between the patient, members of the healthcare team, and physicians.
  • Acting as the liaison with the patient's primary care provider, referral partners and the community to ensure seamless transitions at discharge from the Virtual Clinic to community or clinic and facilitating collaboration amongst all partners;
  • Participating in implementation and evaluation of continuous quality initiatives.

QUALIFICATIONS :

  • Current registration in good standing with the Ontario College of Pharmacists in Part A of the register;
  • Practical knowledge of pharmacology, current best practice clinical guidelines, and ability to interpret lab values
  • Strong clinical assessment skills in identifying and solving drug related problems; experience with CHF / cardiology patients an asset;
  • Excellent medication counseling skills;
  • Demonstrated ability to research and provide drug information effectively to patients and other members of the multidisciplinary team;
  • Demonstrated excellent organizational skills;
  • Demonstrated ability to work effectively within an interprofessional team;
  • Demonstrated effective interpersonal and verbal and written communication skills;
  • Proven ability to exercise independent judgement and to assess situations and problems efficiently and effectively;
  • Previous hospital experience desired;
  • Must have a satisfactory performance record;

  • Demonstrated understanding of and commitment to Lakeridge Health's comprehensive safety programs and practices including staff, patient and environmental safety and participation in regular in-service in this area; and
  • Proven good attendance record with capability of maintaining the same standard.
  • CONDITIONS OF EMPLOYMENT :

    The terms and conditions of employment are in accordance with the Collective Agreement between the Hospital and OPSEU Local 348.

    Successful External applicants must provide a satisfactory Criminal Investigation Reference for the vulnerable sector, dated within the past six (6) months.
