About the Role
The Addiction Support Coordinator’s core responsibilities are to provide direct service to individuals with addiction issues that are made more complex by the presence of one or more of the following concurrent conditions : acquired brain injury, developmental delay, complex concurrent disorders, or geriatric issues. This role will assist with acquiring and maintaining access to medical care, mental health and addictions support, medication management, and all aspects of daily living. The Addiction Support Coordinator also provides consultation and capacity building supports to service providers working with these populations, delivers community education, networks and collaborates with other community agencies and the general public.

Completion of a BSW or other University Degree relevant to mental health, addiction or Social ServicesMinimum one year of progressive experience in addictions or mental health; experience in concurrent disorders or trauma informed practice an assetExperience with case management preferredCSC Clearance conducted by PWGS CanadaStandard First Aid / CPR Level CValid G Driver’s License and satisfactory Motor Vehicle RecordReliable vehicle and willing to transport program participantsMust provide proof from Insurance provider of 1) at least $2 million in general liability and 2) have declared scope of vehicle use to Insurance providerLived experience related to addictions and / or mental health an assetRole Responsibilities
Direct Support Coordination Services
Manage the prioritization and intake of clientsCoordinate and deliver clinical and support services for clients including :
Advocate for and facilitate linkages and access to primary care, addiction, and mental health supports appropriate to the needs of individuals living with addiction who also experience one or more of the following comorbidities : acquired brain injury (ABI), developmental delay, complex mental heath concerns, geriatric issuesConduct, coordinate, and integrate assessments (including GAIN) with the input and recommendations of other health care professionalsDevelop realistic goals and monitor progress towards them in collaboration with clients and identified supports (where appropriate)1 : 1 psychoeducational interventions that support progress towards identified goals and enhance overall health and well-being, particularly in relation to mental health and addictionOngoing assessment of risk and safety with crisis planning and intervention as requiredUtilize evidence-informed practices including (but not limited to) Motivational Interviewing, Stage-Matched Intervention, Trauma-Informed approaches, CBT and DBTMake referrals to internal and / or external program or services, as appropriateComprehensive and collaborative discharge planningConsult and Build Relationships with Community Agencies
Provide consultation, capacity building, and education to service providers working with the target populations to support their ability to work effectively with clients experiencing addictionLiaise with community agencies and referral sources to ensure a steady flow of information and smooth referrals to community supportsBuild and maintain effective relationships in the areas of addictions and mental health as well as primary care, developmental disabilities, ABI, long term care, TC and other related service areasParticipate on community committees as appropriate to support local strategies regarding complex care and reduction of inappropriate emergency department useAdministration and Reporting
Maintain detailed records and ensure reports are completed accurately and in a timely mannerComplete incident reports as requiredPrepare and submit regular travel expense claimsInvolvement in program development, evaluation, data collection / input into CaseWORKSAssist with the development and evaluation of the program as requested
Attend program specific and organizational team meetings as requiredAttend clinical supervision regularlyApplications will be accepted until December 24, 2025 at 4pm.
