Salary Range : $40.36 - $56.30 / hour
St. Joseph's Hospital continues to expand its role in the treatment of complex medical and chronic disease, illness prevention, research and education. Our ambulatory medicine teams specialize in the treatment of complex medical and chronic disease conditions with the goal of assisting individuals to reach optimal health and well-being. Our different teams work together to provide patients with comprehensive assessment, diagnosis, disease prevention and management strategies and follow-up care.
Responsibilities
A challenging opportunity exists for a self-directed and independent Physiotherapist in Medicine Services. The incumbent would be responsible for assessment and treatment of patients with chronic pain, with a consultative approach; patient education and exercise prescription for home programming; collaborating and liaising with an interdisciplinary team to provide holistic and patient centered care; and leading and facilitating patient education sessions.
Essential Qualifications
- Valid registration with the College of Physiotherapists of Ontario
- Minimum 2 years related experience as a Physiotherapist
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Experience working with adults who are experiencing chronic pain
- Experience and demonstrated competency in establishing rehabilitation goals and in the implementation of strategies and protocols for goal attainment
- Demonstrated experience using a patient centered approach
- Capable of working independently as well as collaboratively within an interdisciplinary team setting as well as with case managers, insurance companies and attorneys
- Excellent interpersonal, organizational and strong communication skills, both written and verbal
- Proven flexibility and a high level of initiative
- Knowledge of adult learning principles
- Familiarity with outcome measurement related to clinical practice
Flexibility in adapting to varying case load needs
Demonstrated ability to incorporate evidence based research into practicePreferred Qualifications
Proficiency in French would be an assetKnowledge of computer systems (e.g. workload measurement, Cerner, Microsoft Office)Membership in the Canadian Physiotherapy Association and relevant interest groupsWorking knowledge and experience with theories and principles of guiding adult learning and patient self-management approachesDemonstrates initiatives with program development / improvementExperience and comfort with virtual careExperience with group exercise programming and facilitationTeaching and Research
