Job Description

The Complex Care and Recovery Program is currently seeking a full-time, permanent Social Worker 2 to work in the Psychosis Coordinated Care Service (PCCS). PCCS provides services to clients either who do not require hospitalization but require a highly structured treatment program to transition from an inpatient setting, or who require increased support in the community to prevent decompensation. As a member of an interprofessional team, the Social Worker will provide client-centered psychiatric care and relevant social work services reflective of the needs of the client population. The overall program philosophy incorporates a recovery-based principle, emphasizes education to clients regarding their illnesses, access to treatment and recovery, acute stabilization of symptoms, and promotes progress with continued recovery.

Clients receive individualized, focused, goal oriented support to facilitate stabilization of symptoms in a less restrictive, environment. The program is skill based with intensive group therapy with the goal of stabilizing clients to help them reengage in community activities. Reporting to the manager, you will be expected to function as a primary clinician / case coordinator, work with the interprofessional team, perform assessments, develop, implement and evaluate the interprofessional plan of client care for clients. In addition to offering individual and group counseling support to clients to help them meet their recovery goals, you will work in partnership with clients and their formal and informal supports by proactively engaging them in planning and transitioning into the community. You will have the opportunity to conduct family assessments, offer family education and provide counseling and support to families. You will liaise with other services in the Complex Care and Recovery Program and the community to coordinate care and you will refer and link clients to appropriate services and resources.

You will be able to work with clients from diverse populations; contribute to program development; provide education and participate in the research activities of the program. Candidates must be capable of providing supervision of Masters of Social Work students, collaborating with faculties of social work on special projects related to the development of the social work field, contributing to the development of best practices unique to the social work field and teaching to other professionals. Your skills include the ability to make behavioral observations, mental status assessments, psychosocial interventions, documentation and ongoing communication with other team members. You will have sound knowledge of emergency psychiatry, crisis intervention, addiction issues and severe and persistent mental illness. You will support a workplace that embraces diversity, encourages teamwork and complies with all the applicable regulatory and legislative requirements. Client services will be offered during the day. Evening hours may be required to support family group therefore, day and evening hours will be required. This position is located at 1051 Queen Street West.

Minimum Requirements

The successful candidate must have a Masters of Social Work degree and be a certified member, in good standing, with the Ontario College of Social Workers and Social Service Workers, along with three years’ experience working in a mental health setting. You must possess a strong knowledge of and ability to apply principles of crisis intervention; experience in a mental health / addiction setting; knowledge of recovery; understanding of concurrent disorders and its impact on this population; and an understanding of social work interventions as appropriate for the clients with serious mental illness and their families. You possess knowledge of community supports and resources. You have demonstrated ability to intervene effectively in clinical situations requiring the management of disturbed behaviour and possess effective decision-making and problem solving skills. You also possess superior communication and interpersonal skills and are able to collaborate effectively with stakeholders within and outside CAMH. Demonstrated ability to work with clients of diverse backgrounds is necessary. Bilingualism (French / English) and / or proficiency in a second language would be an asset.
