Community Physiotherapist – Community Advantage Rehabilitation (CAR) – a division of Lifemark.

Durham Region, ON

We are seeking physiotherapists in the following areas of Durham region (Pickering, Ajax, Whitby, Oshawa, Bowmanville, Newcastle, Beaverton, Sunderland, Cannington)

As Durham Region’s largest community based rehabilitative therapy provider we have been providing community rehabilitation services for adults and children since 1997. At Community Advantage Rehabilitation, we work together with our clients to advance their quality of life using best practices, innovation and quality standards. We have gone above and beyond to ensure the safety of our clients and employees with Personal Protective Equipment provided, the spacing of clients, screening, and more. Extensive safety protocols and procedures have been put into place and are mandatory.

We are currently seeking qualified Registered Physiotherapists  to provide services to our Adult clients within the community. This position is a flexible role, and the working hours range from part-time to full time depending on your desired client caseload size.

PT Responsibilities :

  • Provide ministry funded, community based physiotherapy services within the client’s place of residence
  • Perform physiotherapy assessment (through interview, standardized and informal assessment techniques) in the areas of geriatrics, neurology, orthopedics, palliative care
  • Develop and implement goal-oriented treatment plans in collaboration with the client / caregivers
  • Assign patient care plan, and provide supervision and monitoring of physiotherapy assistants when appropriate
  • Provide education to client / caregivers and link to community agencies / resources when appropriate
  • Complete and maintain documentation related to assessment findings, care plans, progress and discharge summaries according to CAR, the referral source and College of Physiotherapists of Ontario standards
  • Requirements :

  • Registered Physiotherapist with the College of Physiotherapists of Ontario (required)
  • Experience in working with in a community setting is preferred but will consider new graduates
  • Ability to work independently and as part of an inter-professional team
  • Ability to prioritize, manage self-scheduling and meet reporting deadlines
  • Excellent oral, written skills and report writing
  • Proficiency with computers
  • Strong interpersonal and organizational skills
  • Reliable transportation is required for this position
