JOB TITLE : Nurse Practitioner (Primary Care)

HOURS : 10-30 hours per week, Monday to Friday

POSITION SUMMARY :

We are looking for an entrepreneurial Nurse Practitioner who loves blending clinical work with program design and quality improvement, you’ll feel right at home here.

GENERAL RESPONSIBILITIES & ESSENTIAL FUNCTIONS :

Direct Patient Care :

• Provide in-clinic care alongside local Gastroenterologists, (episodic primary care, preventative care, chronic disease management, patient education, coordination / transition care).
• Responsible for taking medical history and patient assessment for in-clinic patients
• Support Inflammatory Bowel Disease (IBD) patients pre / post-infusion (assessment, monitoring, symptom management, education, navigation).
• Document thoroughly in the Electronic Medical Records; order appropriate tests / consults; collaborate closely with the interprofessional team.
