Client Care Supervisor - Registered Nurse or Registered Practical Nurse
opportunity to work in the community of York Region and South Simcoe
Remote work with some travel required for client visits.
Full-time Contract, 35 hours per week, 15 Months Contract
Reporting to Client Services Manager Quality Improvement, Partnerships & Client Care, you will be responsible to provide client care needs assessment, case management, supervision and clinical field support for in-home and community health care providers.
This position has an emphasis on in- home services and includes community programs when assessment / case management is required.
This position also provides support in the teaching of special tasks and transfer of controlled acts by exception to Adult Day and Assisted Living Services.
You will be working as a member of the interdisciplinary team, conducting home visits, and teaching & supervising delegated acts to Personal Support Workers.
In addition, you will be providing direct services to clients including needs assessments, ongoing case management, and appropriate interventions in complex care situations to promote safe delivery of care.
You will also foster key relationships with other related agencies and referral sources.
This is a full-time position, 35 hours per week, 8 : 30am - 4 : 30pm - Monday to Friday with occasional work outside of business hours.
What you bring to the role :
- Registered Nurse (RN or RPN) with current certificate of registration with the College of Nurses of Ontario confirmed annually.
- Degree in Nursing or Gerontology Degree preferred.
- Three to five years’ experience with seniors and / or provision of client services in the community.
- Excellent skills assessment, communication, and problem-solving skills.
- Demonstrated teaching, mentoring, and coaching skills.
- Supervisory experience preferred.
- Excellent in risk, crisis, and conflict management.
- Current CPR / first aid certification.
- Proficiency in MS Word, Excel Outlook, and data base applications (GoldCare).
- A valid Ontario drivers’ license and a vehicle are required for this position.
- Ability to work outside regular business hours (8 : 30AM-4 : 30PM) as required.
- Fluency in both official languages (English & French) and other languages, an asset.
- An interest in Infection Prevention and Control (IPAC) and willingness to complete IPAC courses would prove to be an asset to tis role.
