The people we Support:
We serve a remarkable community of individuals with diverse backgrounds and experiences. Since founding in 1987 as part of the Welcome Home program, we've been dedicated to supporting adults with intellectual disabilities as they transition from institutional settings to vibrant community living. We provide 24-hour support to individuals with Autism Spectrum Disorder, FASD, Mental Health challenges, problematic substance use, and behavioral and sensory challenges, ensuring each person receives the care, respect, and opportunities they deserve. Case Manager Job Summary:
The Independent Living Case Manager is responsible for managing an assigned caseload to ensure team members provide appropriate and consistent support to individuals living independently in the community.
Preference might be given to women due to providing direct, one-to-one support to at-risk young women, including individuals impacted by substance use, justice involvement, and complex trauma. The role requires the ability to delivertrauma-informed care in a safe, respectful, and supportive environment.
The Case Manager works collaboratively with individuals, internal teams, and community partners to develop and implement individualized case plans and supportive initiatives.

Compensation & Benefits:
The Case Manager's wage starts at $27.85 an hour.
Hours and Working Conditions:
This is a full-time position with standard work hours 9:30 a.m. to 5:30 p.m., Monday through Friday. To provide additional support outside these hours, you may be required to work some overtime hours with paid compensation.
This position will combine working in a fast-paced office and working in the field to provide ongoing support to the outreach team and the people supported.
Preference will be given to those who reside within the city limits.
