The Role : As an Occupational Therapist in the Primary and Community Care unit, you will assess patients’ needs and develop personalized treatment plans to enhance their independence and overall quality of life. Collaborating closely with an interdisciplinary team, you will deliver holistic care that promotes optimal functional outcomes for patients in a community-based setting.
Responsibilities :
- Conduct comprehensive assessments to develop individualized treatment plans tailored to each patient’s needs.
- Implement therapeutic interventions aimed at improving patients’ daily living skills and functional abilities.
- Collaborate with healthcare professionals, including physicians and social workers, to ensure coordinated and comprehensive patient care.
- Educate patients and their families on therapeutic strategies and resources to support ongoing rehabilitation and well-being.
- Maintain accurate and up-to-date patient records and documentation to reflect treatment progress and outcomes.
Requirements :
Valid certification / license as an Occupational Therapist in the applicable province.Minimum of 2 years of experience in occupational therapy, preferably in a community or primary care setting.Strong knowledge of evidence-based occupational therapy practices and relevant protocols.Excellent communication and interpersonal skills, with the ability to work effectively in a team-oriented environment.Please note that we are exclusively recruiting people who can legally work in Canada.
