The Role : As an Occupational Therapist in Community Rehabilitation, you will work directly with clients to improve their functional abilities and promote independence in daily activities within their home and community settings.
Your role includes assessing, planning, and implementing therapeutic interventions that align with clients' rehabilitation goals, and working in close collaboration with multidisciplinary teams to achieve optimal outcomes.
Responsibilities :
- Conduct comprehensive assessments of clients' physical, cognitive, and environmental needs within community and home settings.
- Develop and implement individualized treatment plans to improve clients' daily functioning and quality of life.
- Educate clients and caregivers on strategies, adaptive equipment, and exercises that support long-term independence.
- Work collaboratively with other healthcare providers, including physical therapists and social workers, to coordinate holistic care plans.
- Monitor and document clients' progress, adjusting interventions as needed to achieve desired rehabilitation outcomes.
Requirements :
- Certified as an Occupational Therapist, with current registration in your province.
- Experience in community rehabilitation or a similar setting preferred.
- Strong knowledge of rehabilitative therapy, assistive devices, and community-based resources.
- Exceptional interpersonal and communication skills, with a focus on empathy and patient-centered care.
- Ability to work independently and as part of a multidisciplinary team, managing time and resources effectively.
