Clinical Pharmacist, Community Pharmacy Programs - Fraser Health Community Fraser Valley

Job Summary
We have a full‑time opportunity for a Clinical Pharmacist to provide pharmacotherapy services to community dwelling clients in Abbotsford, BC. Join our integrated team of four premier health employers.

Responsibilities
- Consult with physicians, nurses, and other health care providers to develop therapeutic plans for each client.
- Individualize client medication therapy by reviewing clinical data and modifying plans as needed.
- Provide medication counselling to clients and families, demonstrating techniques and assessing understanding.
- Assist the health care team in developing drug therapy guidelines and monitor guideline effectiveness.
- Participate in client home visits, program team rounds, and meeting participation as needed.
- Act as a program resource person, providing detailed drug information and supporting staff.
- Provide academic detailing services to physicians to optimize prescribing practices.
- Contribute case studies and review articles to professional journals.
- Provide instruction and / or supervision to pharmacy students and residents.
- Participate in quality improvement activities and medication safety initiatives.
- Manage medication inventories and report procedural errors.
- Prepare reports on various activities and maintain electronic records.
- Perform other related duties as assigned.
Qualifications
Current registration as a Pharmacist with the College of Pharmacists of British Columbia.Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.Valid BC Driver’s Licence and access to a personal vehicle for business purposes.Skills & Abilities
Broad knowledge of organization policies, procedures and standards of care.Knowledge of evidence‑based clinical pharmacy practice, medication use evaluation, research processes and methodology.Ability to apply clinical pharmacy knowledge and make decisions with minimal supervision.Strong communication skills, verbal and written.Capacity to establish and maintain effective working relationships with health practitioners, patients, families and the public.Organizational skills and ability to prioritize work.Ability to operate related equipment and perform required duties.About Fraser Health
