Pay Range
Salary : 65,000 - 68,000 (Annually)
The Care Team Supervisor role combines both clinical expertise and operational / systems proficiency to enable positive outcomes for clients and caregivers, service providers, office staff and funders. The Care Team Supervisor (CTS)'s team includes a mixture of unionized field staff (i.e., PSWs and PS Coaches) at the point of care and office-based staff (i.e., Care Team Scheduling Coordinators).
Key Areas of Accountability
VHA's Care Team Supervisors are required to do the following tasks, but not limited to :
Accountable for the overall operations and people management of their geographical team. This may involve onboarding, engagement, coaching, training (clinical and operational), career development, call / workflow monitoring, monitoring of performance indicators, absence management and performance improvement.
Demonstrates leadership accountability within their region and supports other geographical teams as required.
Ensure that clients receive evidence-informed quality care and population specific quality outcomes, with a focus on client and PSW safety.
Ensures that the team adheres to VHA Home HealthCare policies and procedures, key performance indicators, contractual requirements, Ontario regulatory requirements, health and safety standards and CNO standards of practice. Ensuring fiscal accountability, monitoring resource utilization and resolving rejected billings is an important part of the role as well.
Supervisory oversight of the team of scheduling coordinators, clients within their geographical team and responds to escalated client or PSW issues that may involve joint visits with clients, families, funders.
Required Qualifications :
RN or RPN with CNO registration in good standing.
Membership in professional associations an asset (RNAO, RPNAO, CNA Certification).
Minimum of 3 years experience in community health preferred.
2 to 3 years of supervisor experience in health care strongly preferred.
Strong dedication to working collaboratively within an interprofessional / interdisciplinary care team and across regions.
Demonstrates effectiveness with leading and mentoring staff to achieve desired performance outcomes.
Practices and models client and family centered care.
Demonstrates commitment to professional development and maintaining clinical and leadership competency.
Effective written communication skills and clinical documentation as per CNO.
Knowledge of the Occupational Health and Safety Act, relevant government legislation including Employment Standards Act, the Ontario Human Rights Code, PHIPA.
Computer literacy (i.e., Power BI) and Microsoft Office applications skills required.
Proven skill with data informed decision making and report analysis.
Demonstrated ability to set, monitor and achieve personal objectives and organizational performance targets.
Valid driver’s license and use of a car on the job is a requirement.
