Chronic Disease Educator, (12 months contract) Location : Calgary and Rural Alberta The Opportunity : The Chronic Disease Educator is responsible for delivering chronic disease education (ie. Diabetes, obesity or other identified chronic diseases) based on Clinical Practice Guidelines & Standards of Care. You will educate Physicians, Specialists, and other medical staff (allied health), non-health care professionals and patients on how to better manage chronic diseases. Key Areas of Responsibility : Conduct educational sessions with Physicians, Specialists, Allied Health and Non-HCPs to ensure chronic disease education and knowledge is based on current, evidence-based practice Ensure that education delivered follows the Clinical Practice Guidelines & Standards of Care Implement chronic disease education curriculum and direct overall staff training, programs and educational goals following Chronic Disease Education guidelines. Implements customized educational programs based upon needs Serve as a trusted advisor in identified area(s) of chronic disease education for healthcare professionals Assesses opportunities for Quality Improvement through organization of care Ensure compliance with regulatory / legal mandates as well as Innovative Medicines Canada (IMC) Communicate effectively (verbal and written) with field counterparts and head office to leverage collaborative opportunities while staying within Chronic Disease Education and Novo Nordisk policies and IMC guidelines Responsible for the completion of territory goals. Adjust territory routing to help ensure that providers receive exposure to Chronic Disease Education Team Prioritize budget resources to drive access and deliver education aligned with overall department strategy Executes on Chronic Disease Education tactics and demonstrates adaptability if / when the strategy changes Stays up to date with evolving territory landscape in terms of key clinics, patient access and strategic drivers and barriers of chronic disease management Demonstrate openness to coaching and development and make adjustments based on coaching Involve health systems, employers and other community stakeholders to capitalize on emerging opportunities Work closely with management to share insights about territory dynamics, key opportunities and areas for development in chronic disease education Skills and Qualifications : Bachelor’s degree in nursing (BScN / BSN) from an accredited institution Valid and current RN licensure in the province of Alberta Certified Diabetes Educator is preferred. (must have or be willing to enroll in CDE certification program) Certified Bariatric Educator is preferred. (must have or be willing to enroll in CBE certification program) 5 years clinical experience in the relevant health profession required Chronic disease management experience in specific area(s) identified for the role is an asset Experience working in a multidisciplinary team in the primary care environment Experience facilitating relationships between multiple stakeholders including the local practice, community partners, and allied health care professionals Experience delivering education to healthcare professionals and patients considered an asset Experience utilizing a behavioral change counseling approach is considered an asset Familiarity with patient health records (EMR) considered an asset Demonstrated clinical and assessment skills Demonstrated time management and organizational skills Strong negotiation, conflict resolution and rapport-building skills Proficiency in MS Office (Word, Access, Excel, PowerPoint), email and internet Bilingual (i.e., fluency in English and French) considered an asset
