As a Physiotherapist (Status : Permanent, Full-Time; Location : Sault Ste. Marie; Start date : Immediately) , you provide physiotherapy services to Ontario Health atHome patients to reduce, eliminate, or prevent physical disabilities / impairments and improve the patient’s functional independence and safety in their respective environment.

Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

What will you do?

• Respond to patient referrals from Care Coordinators to properly administer physiotherapy services.
• Assess patient's physical abilities to determine the impact of the patient’s condition on their ability to function in their environment.
• Analyze, interpret, develop, and implement treatment programs based on assessment results.
• Oversee patients progress and adjust treatment programs when needed, to ensure therapy goals are met.
• Assign physiotherapy programming to a rehabilitation assistant based on patient needs and supervise according to professional standards.
• Educate and train patients, their families, caregivers, and / or SDMs, on how to progress with the treatment plan.
• Link patients with other in-house services or outside organizations if needed.
• Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected.
• Accurately document and chart patients’ assessment results, goals, treatment plans, and interventions.
