We are seeking a dynamic highly motivated Intensive Case Manager who demonstrates our values of compassion accountability responsiveness and empowerment; to our clients families and each other.
The Intensive Case Manager works out of the Wesley Youth Housing program which provides service to up to 19 youth who are homeless or at imminent risk of homelessness. They provide case management services assessments and light touch counseling. This is a unionized temporary full-time position ending December 31 2026. It is a unionized position and the schedule is rotating days and afternoons. The rate of pay for this position is $21.62 per hour.
Key Responsibilities:
- Ability to establish a strong positive rapport with a diverse group of youth by being friendly non-judgmental non-intrusive and approachable.
- Oversee the 19-unit program and residents handling issues that may arise within asingle staffed team.
- Engage youth in a meaningful way providing counseling case management information support and advocacy.
- Complete statistics client files daily logs incident reports Serious Occurrence Reports and other administrative documentation as required.
- Complete case manager duties such as intakes assessments discharges complete OW applications ODSP applications housing applications submit landlord letters and direct deposit forms with youth.
- Work collaboratively with community partners and ensure proper communication and sharing of information to better support our clients
- Participate in staff development and team meetings
- Maintain an awareness of other social service agencies in the Hamilton Region and a working knowledge of the legislation that affects the target population of the program.
- Ensure accurate and appropriate case co-ordination is offered and that accurate/updated client files are maintained and that challenging cases/concerns are raised at unit meetings with Manager and/or with case conferencing always with appropriate disclosures on file and legislation adhered to.
What you will bring:
- Minimum of S.S.W. diploma BSW or post-secondary education in a related field
- Member in good standing of the Ontario College of Social Workers and Social Service Workers or a willingness to obtain in the first 6 months
- Minimum 2 years providing direct case management as a prime worker plus minimum 2 years demonstrated staff supervision experience
- Strong communication skills both verbally and in writing. French speaking would be an asset. Must be proficient with Microsoft Outlook Word and Excel with troubleshooting ability.
- Non-violent crisis intervention positive space ASSIST and 1st aid/CPR training is required.
- Ability to work flexible hours including evenings. Schedule is primarily Monday to Friday 7:00 am to 3:00pm and rotates every four weeks to 3:00pm to 11:00pm.
