Shifts : Nights 1800hrs 0600hrs
Status : Permanent Full-Time

Position Summary
Toronto Rehabilitation provides hemodialysis at Bickle Centre site. The unit provides hemodialysis for patients who are post-acute care hospitalization.
These patients are attending Low Tolerance and Long Duration (LTLD), Geriatric rehabilitation or Complex Continuing Care.
We provide both daily and nocturnal hemodialysis.
Duties
The nurse is to provide safe and competent care through application of nursing knowledge, technical aspects of professional practice and demonstrates professionalism and respect.
Responsible for the assessment, nursing diagnosis, integrated care plan development, implementation and evaluation of nursing care.
Participate in professional, program and hospital wide initiatives, including self professional development
- Provides leadership, supervision and mentorship to new staff when needed
- Compliance with and commitment to patients safety responsibilities and corresponding hospital plans, policies and procedures to ensure a safe environment for patients
Qualifications
- BScN
- Member in good standing with College of Nurses
- Nephrology Nursing Certification CNeph preferred
- Current Hemodialysis Experience - preferred
- Member of CANNT
- Excellent patient assessment skills
- Excellent communication, interpersonal, and leadership skills
- Successful completion of Basic Cardiac Life Support ( C level) Heart Saver Plus (Canada)
- Superior clinical decision making skills that reflect patient centredness, problem solving and critical thinking
- Demonstrates knowledge, skill and judgement in the application of hemodialysis
- Demonstrates knowledge and understanding of nursing standards as they apply to the care of patients with chronic kidney disease
- Computer proficiency is required or willing to pursue computer training
- Ability to identify and meet own learning needs for pursuing ongoing professional development
