Reporting to the Director, the Clinical Care Manager (CCM), Patient Access & Flow provides direct leadership to the Patient Flow and Registration Clerks, Discharge Planners, and Social Workers while closely monitoring patient flow metrics to drive improvements. The manager monitors patient movement withing YHC sites, and Patient Transfers and Repatriation with external and Provincial Hospitals and ensures that provincial timelines are met. Collaborates with clinical leaders to develop and implement best practices to improve and maintain patient flow and organizational capacity such as performance measurement boards, access to huddles, and discharge planning.
Knowledge and Skills Required:
The successful candidate will have a Baccalaureate Degree in a health care related discipline. Minimum of 1 to 3 years’ experience in progressively more responsible leadership roles in a healthcare environment, preferably in an acute care academic teaching hospital. Current CPR. Additional education in nursing management, financial management, staff development, Post graduate certification in specialty areas, Masters in Nursing or Leadership and MediTech experience would be desired.
