Clinical Pharmacist

Position Summary
The Pharmacy Team at Vernon Jubilee Hospital is looking for a new Clinical Pharmacist II to join our team! Are you looking to work at the leading edge of healthcare and advance your career? If so, we have a wonderful permanent full-time opportunity for you!

Your Role:
You will provide clinical pharmacy services to patients in acute care and residential care environments. As a member of a multidisciplinary team, you will assess and recommend individualized treatments, serve as the primary resource for drug information, and provide expert advice to healthcare professionals. You will monitor drug prescribing, distribution, and therapy to ensure safe, effective, rational, legal, and economic use of medications. Additionally, you will maintain workload records and statistics, practice patient-centered care by monitoring medical conditions and ongoing drug therapy for effectiveness, compliance, adverse reactions, drug interactions, and other incompatibilities. Your role will also involve communicating pertinent findings and recommendations to healthcare practitioners, maintaining appropriate documentation, interpreting medication orders, and dispensing prescriptions in accordance with legal, professional, and departmental standards.

Qualifications

1. Current registration as a pharmacist with the College of Pharmacists of British Columbia.
2. Certification of completion of an Accredited Canadian Pharmacy Residency program (ACPR1); OR A combination of at least one year of full time equivalent (FTE) recent relevant clinical pharmacy experience and recognized advanced education courses; OR A combination of at least 3 years of full time equivalent (FTE) recent relevant clinical pharmacy experience and an equivalent combination of training, knowledge and experience.
3. Active membership in the Canadian Society of Hospital Pharmacists is encouraged.
