Analysis

Review incident report if delegated, and responds to patient complaints. Review organizational status board for surge status and escalation process. Review EDDs (Expected Date of Discharge) and level of care in Meditech before bed meeting and end of day, if applicable. Identify barriers for discharge and coordinate pending diagnostics and to expedite discharge. Participates in the development, implementation and review of hospital programs, policies and procedures as required. 48_Support Created with Sketch.

Support

Actively participate in and support endeavors to enhance quality care. Provides direction and guidance to all members of the health care team, patients and their families, groups and organizations and collaborates with the health care team members to plan for the care of patients and families. Assist

Assist Patient Care Manager in maintaining professional Nursing Standards and Competencies. Assists in the implementation and evaluation of targets, goals and objectives as identified both internally and externally. Assists with all Quality-Based Procedures (QBPs) within the medicine program; ensuring pathways are followed. Awareness of Key Performance Indicators (KPIs) performance and proficiency. Facilitates a positive learning environment and acts as a resource for staff and health professional students, including onboarding new staff and coordinating placements. Quality assurance

Ensure every patient has a current and accurate EDD and plan of care within Meditech. Promotes the provision of excellent patient care through interdisciplinary collaboration and consensus building, leads care in the unit. Liaises effectively with the Patient Care Manager, Program Director, medical staff, and support staff to ensure the efficient and effective operation of the unit. Identifies patient care system problems and makes recommendations for change.

Qualifications

Possesses a valid Certificate of Competence from the College of Nurses of Ontario. Baccalaureate in Nursing, Health Studies, or Administration. At least 5 years’ acute care or ICU. 3-5 years' leadership experience (Charge Nurse or similar experience). 2-3 years' supervisory experience preferred. Certification in oncology, Coronary Care 1 and 2 preferred. RNAO member preferred. Leadership and teaching/adult education experience preferred. Proficiency

Excellent verbal and written communication and teaching skills. Minimum 5 years of current acute and or critical care experience required. Highly developed critical thinking and decision-making skills. Ability to manage conflict resolution and negotiation at all levels of the organization.
