About the CompanyOur client is dedicated to providing high-quality client and family-centered care is seeking for a Supervisor of Client Services(RPN) who will be working within a multi-disciplinary team and is responsible for conducting in-home client assessments of client needs, providing care coordination, and supervising a team of Personal Support Workers.
Responsibilities:- Supervising, coaching, training, and supporting the PSWs to provide high quality client and family centered care
- Lead and direct a PSW team to deliver a value-added seamless service experience that meets clients’ needs and expectations with delegated acts training
- Temporarily, assist operations with the supervision of other sites to cover leave of absence
- Conduct initial and ongoing in-home client assessments/reassessments of clients' needs for client centered service according to Agency guidelines
- Develop care plans based upon client’s health and functioning level, support network and home environment
- Process referrals where required
- Monitors workflow and conducts in-home spot checks while communicating with employees
- Maintains accurate documentation on supervisory matters (e.g., staff absence/lateness, client complaints, WSIB claims, etc.)
- Ensures the fair and equitable treatment of all individuals
- Inputs and verifies PSWs schedules into AlayaCare
- Completes timekeeping including approving time off, sick time, float day, lieu time and vacations
- Conducts performance evaluation of PSWs
- Attends internal and external meetings, trainings, conferences, and health fairs
- Collects information on new clients, discharged/terminated clients, length of service, age of client and type of service provided
- Complete monthly health and safety inspections for the applicable site
- Oversee smooth operations of Assisted Living sites which will also include maintaining good working relationship with the housing providers and ordering office supplies
- Maintain and process Assisted Living referrals of waitlist through Health Partner Getaway
- Regularly create and update coordinated care plans for clients in the Client Health and Related Information System (CHRIS)
- Participates in committees, staff discipline meetings and in job interviews
- Demonstrate a positive professional attitude when representing CANES Community Care
- Prepare statistical reports and responsible for meeting targets of the program
- Alternates 24-hour on-call responsibility on a rotation basis
- Responsible for emergency preparedness plan of the program and implements it in times of emergency
- Contribute to a culture of client safety and employee Health and Safety, by complying with all safety practices of the organization, reporting unsafe practices and adverse events, and acting on safety recommendations
Qualifications:- RPN designation required
- Must be registered and in good standing with the College of Nurses of Ontario
- Ability to work remotely with minimal supervision
- Ability to work with diverse communities
- Excellent verbal and written communication skills and assessment skills
- Strong attention to detail and organizational skills
- Knowledge of computer programs i.e. interRAI Community Health Assessment Tool, Screener, AlayaCare, etc.
