Remote RN Oncology Case Manager - Government (VA) Contract
Position Summary
The Registered Nurse (RN) Care Manager in Primary Care is a team member who provides comprehensive and coordinated nursing care to an assigned panel of patients and who collaborates with VA and community services to effectivity meet the health promotion or disease prevention, acute, chronic, and long-term needs, based on the Veteran's goals and plan of care.
This role will primarily assist in Oncology and Hematology.
Contract
This role will be telehealth assisting patients out of the Durham, NC Veteran Affairs Medical Center. The contract is for 1 year with the potential to extend for up to 2 years in total.
Schedule
Day Shift.
Remote Eligibility
This position is remote eligible as long as you are residing within one of the 50 US states or district of Columbia.
Responsibilities
- Assess the health care needs of the patient and maintain or develop a plan of care to meet interdisciplinary goals
- Assess and act on significant changes in the patient’s health condition
- Coordinates care and provides direct patient care and patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals.
- Coordinate and participate in admission, transfer, and discharge of patient
- Demonstrates leadership by serving as a Patient Aligned Care Team (PACT) leader and serves as a preceptor or mentor to colleagues or students to enhance professional growth, development and practice.
- Educate patients and / or families regarding health and diagnoses and medication administration.
- Care Coordination and Integrated Case Management model, providing Care Coordination and Case Management (CC&ICM) to complex patients.
- Comprehensive care for Women Veterans, vaccination initiatives and other clinical services as required.
- Ability to perform accurate physical assessments, triage patient needs, and make sound, independent clinical judgments resulting in appropriate plans of care and improved patient outcomes.
- Organize and prioritize their own work and interface with resources and programs in the community including co-managed care providers to coordinate patient care across the continuum of care including but not limited to non-VA care, inpatient hospitalizations, specialty care, palliative care settings.
Requirements
- Bachelor of Science in Nursing.
- Graduate from a college or university accredited by National League for Nursing Accrediting Commission (NLNAC), or The Commission on Collegiate Nursing Education (CCNE).
- Basis Life Support (BLS) and Advanced Cardiac Life Support (ACLS) through the American Red Cross or American Heart Association.
- At least 3 years of experience with case management.
- At least 3 years of experience with hematology / oncology.
- Current, full, active, and unrestricted license for any state or commonwealth to practice as a Registered Nurse. Depending on the contract, state licensure for in the state practicing may be required.
- U.S. Citizenship : Shall be a U.S. citizen.
- Proficiency in EMR / EHR systems and general technology.
- Successful completion of any necessary federal background check and VA credentialing polices.
This job is CONTINGENT upon contract award, client approval, completion of a favorable background investigation, and the ability to obtain and maintain a Public Trust / High Risk clearance.
