Join to apply for the Registered Nurse (RN) - Seniors Clinic role at Sherwood Park - Strathcona County Primary Care Network. The SPPCN is seeking a dedicated and compassionate Registered Nurse (0.6 FTE) for a unique and rewarding position in our Seniors Clinic. You will work with a multidisciplinary team to provide comprehensive, patient-centered care for older adults, supporting healthy aging, cognitive screening, chronic disease management, and proactive care planning. This position offers a choice of schedule:
- Monday, Thursday, Friday
- Tuesday (12:00–8:15 pm), Thursday, Friday
Responsibilities
- Assessment of patient conditions, needs and goals in collaboration with the patient, attending physician and interdisciplinary team
- Identify needs and set priorities in collaboration with the client and other team members
- Provide short term interventions as needed
- Facilitate appropriate referrals to community resources and programs
- Provide relevant patient education to prevent and manage clinical conditions
- Participate in evaluation of the clinic and its services
- Assist in planning, developing, and monitoring for services provided
- Assist in the development of policies and protocols for chronic disease management, geriatric care, maternal/child and mental health
- Develop systems and communications protocols to facilitate patient follow up
- Assist in setting up health promotion programs relevant to the needs of the practice population
- Assist in developing and implementing patient self-management programs
- Assist in implementing strategies for improving the quality and provision of care for the practice population
- Any other duties reasonably required for the success of the PCN
Education & Experience
- Baccalaureate degree in Nursing preferred
- Five years or more of related nursing experience
- Current CRNA Registration, CPR, and BCLS
- Initiative, accountability, decision-making, interpersonal and communication skills
- Ability to work in a dynamic environment with flexibility and balancing priorities
- Experience in PAP testing
- Experience working in a team with other health professionals
- Advanced skills in caring for patients with chronic, complex, and/or specialized needs
- Experience establishing protocols for program startups and workload management
- Knowledge of nursing processes and practices including intervention strategies and therapy
- Strong written and oral communication, high level of perceptiveness
- Critical thinking skills
- Ability to build working relationships with healthcare professionals, clients and community members
- Ability to prioritize and manage time in a busy environment
- Respect and understanding of confidentiality
- Knowledge of medications and ability to conduct medication reviews
