POSITION SUMMARY & PURPOSE
The LPN will work collaboratively with the community health nurse, Elders, and other health professionals to provide healthcare services, and support to Xaxli’p members. The ideal candidate will have a strong understanding of Indigenous health perspectives and a commitment to culturally safe care.
KEY RESPONSIBILITIES
- Provide nursing care, including assessments, treatments, medication administration, and wound care.
- Collaborate with physicians, nurse practitioners, and other healthcare providers to coordinate care.
- Conduct home visits and wellness checks for Elders and community members in need.
- Maintain accurate and confidential client records following healthcare regulations.
- Advocate for culturally appropriate healthcare services and ensure respectful, trauma-informed care.
RESPONSIBILITIES
Completes and submits service delivery data and reports as required by FNHA Home and Community Care Program.Identifies community members requiring home care support.Assists in direct nursing care as required with and for clients to achieve optimum health in various situations : chronic disease, acute illness, injury wound care, and palliativecare.Coordinate home care services in such a way to meet the home care service needs of the clients while encouraging independence, family, and community support.Completes a structured client assessment that includes ongoing re-assessment and determines client need and service allocation.Provides clients and family caregivers with health information as required through demonstrations and informal teaching sessions.Participates in case management and family conferences as appropriate.Ensures that clients admitted to the Home and Community Care Program have access to medical supplies and equipment.Provides mentoring, direction, guidance, support, and supervision for Xaxli’pHome Support Workers.Establishes internal and external linkages with other professionals and community resources.Perform other related duties as required.QUALIFICATIONS
Education, Training and Experience
Licensed Practical Nurse (LPN) certification, active registration and good standing with the BC College of Nurses and Midwives (BCCNM).Minimum of 1-2 years of nursing experience (preferably in primary care).Experience working with Indigenous communities and an understanding of traditional healing practices is an asset.Current CPR and First Aid certification.Valid driver’s license and reliable transportation.Ability to pass a criminal record check, including vulnerable sector screening.Knowledge, Skills and Abilities
Knowledge of St’at’imc Culture, Language and Healing Practices.Excellent oral and written communication and comprehension skills.Active listening and critical thinking skills.Ability to function independently and in a team environment.Demonstrated organizational and time management skills; ability to work effectively under pressure and meet deadlines.Good knowledge of the vision and values of Xaxli’p and the community overall.Ability to maintain positive working relationships with staff, community members and partners.WORKING CONDITIONS
